I wish to register my 1971 Malibu with SS option

1. Readable photo of Fisher Body number
plate (also known as 'Trim Tag', 'Cowl Tag',
and/or 'Firewall Tag') like the example below.
The Fisher Body number plate is located on top
of the firewall just above and to the right
of the master brake cylinder. 2. Readable
photo of VIN plate if possible; or photo of
VIN from title, warranty Protect-O-Plate card,
or other form of vehicle identification showing
the complete VIN. The VIN plate is found on
the driver's side top of the dash and visible
through the windshield.. 3. Photo of
the car. Preferably a three-quarter front view
with as little background clutter as possible
and minimum resolution of 640x480.
4. Since the SS option was just that,
an option and not a series, any documentation
showing the Z15 option such as build sheet,
dealer paperwork, etc. showing the option and
matching VIN.

03/28/2016: If you are using a smartphone of some
type to complete the form, it may not be forwarded
by the email handler on your phone. If you experience
a problem, like me not getting the completed form,
and insist on using a smartphone then please take
a screenshot of the completed form BEFORE you click
the "Submit" button and email me that
screenshot plus any photos you have to document
the car and/or wish to submit to the gallery.
I've experienced the same issue with some other
email address as well from MACS and PCs, not getting
the registration form. If you received your confirmation
email but do not hear back from me within a couple
of days of registration, please forward your copy
of the confirmation and attach any photos to the
forwarded email and email them to me at
Try to find a photo of your Chevelle that does
not have the front or rear cut off like this:
Take a few steps back and capture the entire car.

Notice: I have been experiencing problems with my third-party
form handling supplier and sometimes the registration form does
not get sent to the proper address of the registry. If you do
not hear from me or see your car in the registry after a couple
of days, please forward your response copy of the form and any
images submitted to email address above.
